Dental Membership Plans

Individual Plan:
($349 adult / $299 youth annually)

• First year – one comprehensive exam & one periodontal exam

• Subsequent year – one annual exam & one periodontal exam

• Two cleanings (in absence of infection)

• One oral cancer screening

• One fluoride treatment

• 4× bitewing x-rays

• One panoramic x-ray

• Individual x-rays (as needed)

• 50% off additional cleanings

• 15% off fillings, core build-ups, oral surgery, root canals, crowns, bridges, partials, and dentures

• 10% off veneers

• $500 off clear aligners

Family Plan:
(+$249 per additional family member annually)

Exclusions and Limitations:

This program is a discount plan, NOT a dental insurance plan.

It cannot be used:

• In conjunction with another dental plan, dental insurance plan, or financing program such as Care Credit unless it is discussed with the doctor beforehand.

• For treatment which, in the sole opinion of our doctors, lies outside of the realm of their capability

• For referrals to specialists

• For hospitalizations or hospital charges of any kind

• For costs of dental care covered under automobile or medical insurance

• For services of injuries covered under worker’s compensation

Membership is valid for one year from date of enrollment.
Membership fees are non-refundable and must be paid in full at time of enrollment.
Plan is only honored at Saint Louis Dental Studio. It cannot be used at any other dental office

Periodontal Plan:
($649 annually)

• All services and offerings included in the individual plan

• Three cleanings (periodontal maintenance)

• 20% off scaling and root planing

• 50% off Arestin placement
